2. Fill out this Shipping & Payment Authorization form. You can fax it back at 336.855.6566 or call 800.603.8107 with your information.

For North Carolina businesses, please complete this Sales Tax Certificate of Exemption and fax it to 336.855.6566 or mail it in.

3. As soon as you receive an e-mail from us, begin uploading to your new account.
